What is my contribution report deadline?

All contribution reports are remitted through the I-Remit Portal. To determine your contribution report deadlines, please refer to your Collective Bargaining Agreement (CBA). If you do not have a copy of your CBA, contact your Business Agent. If the Fund Office does not receive contribution reports and payments by the deadline, the employee will not be considered eligible for coverage until payment has been received and processed by the Accounts Receivable Department.

What should I do if I have a new employee or a current employee that is retiring, is terminated or resigns?

When a company hires a new employee or when a current employee retires, is terminated or resigns, the Fringe contact must complete an Employee Information Form and fax it to the Accounts Receivable Department at (708) 482-3756.

What does my employee need to know about adding a dependent to their health plan?

If your employee has a Life Changing Event such as getting married, having or adopting a child, or gets a divorce, they have 90-days from the date of the event, to submit the required documents to the Fund Office to validate their dependent(s).

If the employee fails to submit their required documents (please see below) during the 90-period, their dependent(s) will not be added until the next open enrollment, unless they have a qualifying Life Changing Event.

 

The following documents must be sent to the Fund Office. The employee can either fax it to the Benefits & Eligibility Department at (708) 352-3310 or upload the information through their My150 account.

What should my employee do if they perform work outside of Local 150's jurisdiction?
