A Schedule of Benefits explains what services are covered, the associated costs like copays, deductibles, and coinsurance, and any limitations or exclusions that are included, to give you an idea of what you’re going to pay for medical treatments and procedures.

Plans’ benefits are subject to eligibility, maximum Plan benefits, reasonable and customary determinations (or negotiated fees for PPO dental services) and any special limits noted in the Plan. Charges that exceed the reasonable and customary amount or other Plan limitations will not be considered eligible in determining Plan benefits. To avoid additional out-of-pocket costs, be sure to utilize in-network providers.

Under the active Welfare Fund, there are numerous health plan options available depending on the Collective Bargaining Agreement (CBA) that you fall under.

Below you can find the details of your specific health plan option. You can also find copies of your Schedule of Benefits and Summary of Benefits & Coverage (SBC) on your My150 account under “My Plan Documents.”

MOE Health Plan Marketplace (Hourly Members)

Hourly Members of the MOE Health Plan Marketplace – Each year during the annual open enrollment period, you are able to select your health plan option.  These options include Plan A PPO, Platinum PPO, Gold PPO, Silver PPO, EPO Plan, OHC Plan.   Each of these health plan options have various fees associated with each Covered Service.

Municipalities (Monthly Members)

Monthly Members which are Municipalities – Each year during the annual open enrollment period, you are able to select your health plan option.  These options include Plan A PPO or the EPO Plan.   Each health plan option have various fees associated with each Covered Service.

Owner-Operator or Relative (Monthly Members)

Monthly Members which are Owner-Operator or Relatives – Each year during the annual open enrollment period, you are able to select your health plan option.  These options include Plan A PPO, Platinum PPO, Gold PPO, Silver PPO, EPO Plan, OHC Plan.   Each of these health plan options have various fees associated with each Covered Service.

Non-Marketplace Legacy Plans (Monthly Members)

Monthly Members of Legacy Plans – For this group of members, your CBA dictates which health plan option you fall under:

  • Plan A-2: City of Chicago
  • Plan A-3: Non-Bargaining Employees
  • Plan A-5: Municipality
  • Plan B: Monthly Plantsmen
  • Plan C: COBRA
  • Plan E: Monthly Landscapers
  • Plan F: Monthly Staff Members

What is the Consumer Coverage Disclosure Act?

Requires any employer who provides group health insurance coverage to employees in Illinois to provide all employees with a list of essential health insurance benefits regulated by the State of Illinois; as well as a comparison of which of those benefits are covered by the insurance plan that is provided by that employer. This also applies to employers whose group health insurance plan is not regulated by the State of Illinois. Employers must provide this information to all employees upon hire, annually, and upon request.
