The Schedule of Benefits is a document that outlines the fees associated with each type of health care service covered by your plan. This includes: Your deductible and out-of-pocket max amounts.

The Summary of Benefits and Coverage (SBC) is a snapshot of a health plan’s costs, benefits, covered health care services, and other features that are important to consumers. SBCs also explain health plans’ unique features like cost sharing rules and include significant limits and exceptions to coverage in easy-to- understand terms. Members will also have a uniform glossary of terms commonly used in health insurance coverage such as “deductible” and “co-payment” to help them understand some of the most common but confusing jargon used in health insurance.

Under the active Welfare Fund, there are numerous health plan options available depending on the collective bargaining agreement (CBA) that you fall under.

Below you can find the details of your specific health plan option.

MOE Health Plan Marketplace (Hourly Members)

Hourly Members of the MOE Health Plan Marketplace – Each year during the annual open enrollment period, you are able to select your health plan option.  These options include Plan A PPO, Platinum PPO, Gold PPO, Silver PPO, EPO Plan, OHC Plan.   Each of these health plan options have various fees associated with each Covered Service.

3-Tier Municipalities (Monthly Members)

Monthly Members which are 3-Tier Municipalities – Each year during the annual open enrollment period, you are able to select your health plan option.  These options include Plan A PPO or the EPO Plan.   Each health plan option have various fees associated with each Covered Service.

Legacy Plans (Monthly Members)

Monthly Members of Legacy Plans – For this group of members, your CBA dictates which health plan option you fall under:

  • A-2: City of Chicago
  • A-3: Non-Bargaining Employees
  • A-5: Municipality
  • B-1: Monthly Plantsmen
  • C-1: COBRA
  • E-1: Monthly Landscapers
  • F-1: Monthly Staff Members

What is the Consumer Coverage Disclosure Act?

Requires any employer who provides group health insurance coverage to employees in Illinois to provide all employees with a list of essential health insurance benefits regulated by the State of Illinois; as well as a comparison of which of those benefits are covered by the insurance plan that is provided by that employer. This also applies to employers whose group health insurance plan is not regulated by the State of Illinois. Employers must provide this information to all employees upon hire, annually, and upon request.
